RETURNED CHECK- client will be responsible for full amount due for services and product plus a $30.00 insufficient funds fee.
CANCELATION FEE- client will be responsible for paying a cancellation fee of $25.00 with less than 12 hour notice. The fee will be $50.00 for cancellation of hot stone massage with less than 12 hour notice. This is an appointment based business. Voice mail is available to assist clients in giving enough notice to cancel and reschedule appointments.
APPOINTMENTS- Hours of operation are as follows (subject to change when needed)Monday-Saturday 9a.m., 10:30a.m., 12:00 noon, 1:30p.m., 3:00p.m, 4:30p.m., 6:00p.m., and 7:30p.m., Saturday last appointment is scheduled at 3:00p.m.. Closed Sunday. Advanced notice for most appointments are required. Therapists are scheduled to work, this is not an out-call office. Events for health fairs and corporate events require at least four weeks advanced request.
REFUNDS- There are no refunds available for massage therapy. Returned products must be in original condition, unopened, with original tags not removed in order to receive a refund. Items purchased online such as Juice Plus products need to be returned to manufacturer and unopened for full refund. Clients are responsible for any shipping costs of returned products to manufacturer or Sedona Massage Co.as well as a 10% restocking fee.
GIFT CERTIFICATES- Gift certificates may be purchased at office during appointment visit or online from the shopping cart option. Gift certificates are good for six months from the date of purchase and are non transferable.
IDENTIFICATION- Sedona Massage Co has the right to request identification in the form of picture id when interviewing prospective client, processing client intake forms and insurance forms. Driver license number is requested when paying by personal check. Insurance cards will be photo copied to process medical claims when applicable.
SERVICES DENIED- Sedona Massage Co. reserves the right to refuse services to any prospective client. Massage is a physical contact service. In some cases, several health conditions are contraindicated for massage. |
